What Is Specialist Support Coordination?
Specialist Support Coordination is a higher level of support provided to participants:
- With complex disabilities, psychosocial issues, or multiple diagnoses
- Involved with justice, child protection, mental health, or housing systems
- Who need help during critical times (e.g., hospital discharge, homelessness risk)
Our experienced Specialist Support Coordinators are qualified professionals (e.g., social workers, mental health clinicians) who can work through complex environments to get you the support you need.

- Manage crisis situations and reduce risks
- Coordinate multiple services (mental health, housing, justice, etc.)
- Establish a stable support network
- Resolve barriers to service delivery
